What to ask: “After execution, how do we prove it did what was approved—and catch divergence fast?”

Plain English: If a workflow doesn’t hit the gate, it isn’t governed.
- What it is: A measurable map of governed vs ungoverned execution paths, and a plan to close bypasses.
- Why it exists: The biggest risks live in shadow integrations and “one-off” workflows.
- Failure mode: Controls exist, but real actions execute on an unwired path; governance becomes a dashboard watching the wrong hallway.
- Evidence signal: Coverage map + bypass detection (“these high-risk actions never pass through the commit boundary”).
What to ask: “Show me the workflows that can still file/send/approve/move without a verdict.”
